The focus of PATH is to provide outreach and services to individuals with serious mental illnesses and those with co-occurring substance use disorders who are homeless or at imminent risk of becoming homeless, with priority provided to those who are literally homeless. The PATH team routinely conducts on the street outreach in the community to identify those in need who are not linked with traditional services and resources.
PATH programs are considered a "front door” to homeless services funded through the HUD Continuum of Care (CoC) and to mainstream mental health, primary health care and substance abuse service systems. PATH staff also provide assistance to PATH program participants to coordinate pursuit of entitlements and benefits, as well as housing and other community mainstream resources. This program is designed to provide a network of supports to the program participant to assist the individual in transitioning into housing successfully.
