Macomb County Community Mental Health (MCCMH) - Intake, Services and Supports

In order to live more successfully in an integrated community, some individuals with a disability find they need the support of their family and friends, or what is called natural supports. However, some individuals have limited natural supports and may need to link within the community to find support, or what is called supplemental support. MCCMH serves as a bridge to close the support gap for those who have limited natural supports, or would like additional support outside of their family and friends.

The services and support provided by MCCMH allow the person to become a part of the community. The goal of MCCMH is to provide an array of quality services, which promotes community participation, self sufficiency, and independence.

MCCMH serves Macomb County residents, and is the manager of the public mental health services and supports in Macomb County, including substance abuse treatment. The qualifications for services are:

  • Be a Macomb County resident
  • Have a diagnosis of a severe and persistent mental illness
  • Have a diagnosis of a developmental disability; developmental delay, or a history of such a disability.
  • The person may believe that he/she has a disability, but has not been tested.
  • Your child may have one of these conditions.
  • A family member or individual may believe that there is a substance abuse issue that should be addressed.
  • Have Medicaid, MIChild, or other health care sponsored by the State of Michigan.
  • Not having insurance, but Medicaid eligible.

Assessments and Screenings

The Access Center staff will conduct assessments and screenings that will decide if the individual is eligible for publicly provided mental health or developmental disability services.

Substance Abuse

If a person or family member is concerned about substance abuse treatment, and would like to receive services through MCCMH, contact Community Assessment, Referral, and Education (CARE). The CARE staff will schedule an office appointment for an assessment. During that time, the CARE staff will assess if the person is qualified for publicly funded substance abuse treatment.

Planning Your Services

MCCMH goal is to provide an array of quality services, which promotes community participation, self sufficiency, and independence for each person and/or family that participates with CMH.

The process used to create and provide specific services to the individual and/or family is called Person Centered Planning (PCP). PCP is based on the person's Dreams, Goals, Desires, and Strengths. PCP builds on both the individual's current goals and future dreams, which will build on the ability of the person to be a part of the community

Children receiving MCCMH services are also entitled to be a part of their planning process based on the child's interest and ability. The planning process often involves the entire family and/or guardian(s). Because planning often involves the family/guardian(s), this is called Family Centered Planning, which will involve goals for the child and hopes and needs for the entire family.

In addition to PCP and Family Centered Planning, Self-Determination is a model used service delivery that allows the person receiving mental health or developmental disability services to direct the purchase of your approved services using a fixed amount of Medicaid dollars. The person's Individual Budget or the funds that are available are based on the person's PCP. Upon qualifying for services, the ACCESS Center will provide detailed information on how to implement Self-Determination.

Clubhouse Programs

Members and staff work together to operate the clubhouse. The clubhouse focus is to encourage participation in the community, fostering recovery and social supports.

Drop-in-Centers

Drop-in-Centers are a peer operated program and provide a variety of support services, including clothing, socialization, assistance with permanent housing, and the support of starting and continuing mental health services.

COMMUNITY HOUSING NETWORK (CHN)

History: CHN began operations in 2001 when a staff of three embarked upon a local initiative to create and manage housing resources for people with disabilities. As a 501 (c) 3 non-profit, CHN contracted with Macomb County Community Mental Health Authority to address the housing needs of persons with disabilities. Three areas were discussed:

  • To address the escalating cost of group home leases and to provide property management for nearly 180 licensed group homes.
  • To create alternatives to group homes by generating and managing various assets. CHN has begun to reconfigure existing leasing arrangements with for-profit property owners, and has begun to address the issue of runaway rents and lost equity. The organization has purchased some group homes, in part by obtaining assistance from the Federal Home Loan Bank Indianapolis and plans to continue acquiring properties to build critical equity.
  • A central point where people can obtain information about affordable housing and available resources. The result was the Housing Resource Center (HRC).

Community Housing Network works in collaboration with Macomb County Community Mental Health to provide supported housing and a Housing Resource Center to members of MCCMH consumers. CHN links with all service providers within MCCMH by seeking and creating safe and affordable housing opportunities and information.

CHN works in partnership with support coordinators, agencies, and circle of supports to develop individualized, creative solutions to the housing needs of consumers. We believe that people with disabilities will:

  • Attain safe, decent, affordable housing of their choice.
  • Have control of which the person lives with, if anyone, including other people with disabilities and/or people who do not experience a disability.
  • Housing choices are separate from the attainment of other supports or services.

CHN facilitates the utilization of local, state, federal and private resources to promote sustainable, affordable housing opportunities in Macomb County.

Office of Recipient Rights

Community Housing Network and Macomb County Community Mental Health provide services under HIPPA, which requires all information must remain confidential, unless
the consumer has signed a Release of Information (ROI). The Office of Recipient Rights (ORR) can help with the understanding of how confidentiality works for the consumer, family, and supports.
