Permanent Supportive Housing Programs
We help people with disabilities and chronic health conditions who have experienced homelessness find long-term housing and wrap-around services to help them stay in their homes.
Why Permanent Supportive Housing?
- Available data reinforce evidence that permanent supportive housing is an effective approach for meeting the needs of people with disabilities and chronic health conditions who have experienced homelessness.
- Research indicates that this combination of long-term housing and wraparound services leads to improved residential stability and reduction in psychiatric symptoms.
- Studies have also found that supportive housing is associated with significant reductions in costs for emergency room visits, hospitalizations, shelters, sobering centers, jails, and other public services used by people experiencing homelessness.
Who Is Eligible?
To qualify for Permanent Supportive Housing (PSH), applicants must be literally homeless (Category 1 per HUD) and have a documented disability. Potential participants for PSH are identified using an assessment and screening process defined by the local Continuum of Care (CoC). When a slot becomes available, CoC members use a prioritization protocol to determine who will be offered the slot. Persons who are chronically homeless are prioritized for all slots.
CHN currently offers PSH to residents of Oakland, Macomb and Wayne County (except for the communities of Detroit, Hamtramck and Highland Park).
